Accidents don’t just happen. They are almost always caused by the actions
or inactions of one or more people. Most
accidents are not beyond our control. They
are preventable, and avoidable, but yet they can happen.
Jus as the actions or inactions of
people caused accidents to happen, their actions can prevent it from
happening. Only people can prevent
accidents from happening. O work
practices, rules, training or even great equipment can prevent accidents from
happening.
Your own safe behavior
Is your own greatest safeguard
Safety begins with management, and
it requires us to take control of safety in the workplace, to recognize the
hazards that exist, and to work to control them.
An accident is defined as unplanned
event which is undesirable, unexpected and no control. Accidents do not necessarily result in
injury. There are a few theories of
accidents:
- Domino theory
- Accidents occur as a result of an unsafe act by a person and / or mechanical hazards
- Unsafe act or unsafe condition does not always immediately result in an accident or injury
- Human factor theory
- Inappropriate activities
- Human error + overload
- Inappropriate response
- Accident / incident theory
- Extension of human theory
·
System failure
·
Overload (human)
·
Ergonomic traps
(incompatibility)
·
Decision to err
- Epidemiological theory
- Causal relationships between environmental factors and disease
- System theory
- Depression and accidents causation
- Management failures and accident causation
